Investigation Summary
At 6:00 p.m. on May 29, 2018, Employee #1 was unloading an air freight container that was on a flatbed trailer. The employee was standing on a platform, loading the parcels onto a conveyor. The truck driver unhitched from the trailer and it dropped slightly, pinning part of the employee's left foot between the dock and the trailer, and amputating his big toe. Employee #1 was hospitalized for his i njury.
