Investigation Summary
At 2:45 a.m. on March 10, 2018, an employee went to assist a coworker with opera ting the carpet separator and placed his hand into an unguarded portion of the m achine. The employee amputated his middle finger from his right hand when it wa s caught by a rotating part on the rotary lock system and was hospitalized.
