Investigation Summary
At 3:00 p.m. on March 7, 2018, two female employees were working for a firm that manufactured storage batteries. Employees #1 and #2 were, in different instances, inspecting material and equipment as batteries were being compressed together. The compressing equipment was known as the MMAMC 2.0 "Picture Frame" cylinder. The employees were leaning on a picture frame as the cylinders were being compressed. They inserted their fingers into an unguarded hole which housed a cylinder that moved laterally past this hole. Employee #1 sustained the laceration of her middle finger. Employee #2 sustained the amputation of her ring finger at the first knuckle. One employee was hospitalized, and the other employee was not hospitalized.
