Investigation Summary
At 1:40 p.m. on February 28, 2018, an employee was performing preventative maintenance when the air fitting came out of the valve the actuator came down on the employee's right index finger. The employee amputated his right hand's index finger between the first and second knuckle. The employee was hospitalized.
