Investigation Summary
At 2:40 p.m. on January 24, 2018, Employee #1 was performing maintenance activities of the metering cylinders located in the Packaging Robot cell on line #1. Employee #1 operating inside the cell, was performing an operational test of the metering cylinders escapement pins. Employee #1 was providing verbal instructions to a coworker who was manually initiating the cycle via solenoid control outside the cell. As Employee #1 was making adjustments to the float controls, his left third finger dropped between the rear plate of the escapement pin and the cylinders housing. The coworker initiated a cycle, and Employee #1's fingertip was caught between the two components.
