Investigation Summary
At 12:45 p.m. on December 7, 2017, an employee was repairing a die on his work bench. The employee used large "C" clamps to pick up a die that weight approximately 1500 pounds and was placing dunnage under the die. The employee caught his finger between the die and the dunnage when one of the clamps slipped. The employee amputated his middle finger at the first knuckle and was hospitalized.
