Employee Sustains Partial Amputation Of Two Fingers By Rotat

At 3:00 p.m. on October 13, 2017, Employee #1, a cement plant operator, was manu ally cleaning the screen portion of one of the terminal loading stations. To cle an the screen, the employee had to open the access door and use a combination of a hoe tool and his hands to scope out clumps of dry cement material that could be blocking the loading terminal screen. There was a dust collector located abov e the screen. The dust collector had a rotary air lock device placed between th e screen and the dust collector. While cleaning the screen, the employee did no t lockout the rotary air lock device of the dust collector. The rotating blades of the rotary air lock were not guarded to prevent contact with the blades. The employee's right fingers contacted the rotating blades, causing partial amputat ion injuries to his right index and middle fingers. Employee #1 was transported by EMS and was hospitalized for his injuries.
