Investigation Summary
At 10:46 a.m. on October 3, 2017, Employee #1 was preparing the stacker. When he had completed the task, he exited the stacker and walked around to the operator's side. The employee put his hand on a moving shaft. It caught his arm and pulled it back towards him. Employee #1 suffered a compound fracture to his left forearm and was hospitalized.
