Investigation Summary
At approximately 4:30 p.m. on January 3, 2005, an employee, a supervisor and a foreman, was finished repairing a portion of an existing roof when he slipped and fell from a ladder onto the first story roof then rolled off the roof and fell to the ground bushes below, a total of 14 ft. He was hospitalized and treated for fractures of his leg and wrist.
