Investigation Summary
On November 19, 2004, Employee #1 was working for his employer, a building products retailer which delivers roofing materials to work sites using trucks and conveyors. Employee #1 was unloading bundles of shingles while a coworker was loading the material to the conveyor. Employee #1 was then unloading bundles of shingles and stacking them onto a roof with a 5:12 pitch. At approximately 9:45 a. m., Employee #1 was on the roof when he slipped on a piece of wet plywood sheeting, falling 10 feet to the concrete level below. Employee #1 sustained a head injury and multiple fractures. He was hospitalized for three weeks. The incident was reported on April 4, 2006 to the California Department of Occupational Safety and Health.
