Investigation Summary
On November 8, 1994, employees of Oscar Larson started an excavation. On November 11, 1994, Employee #2 and a coworker arrived to complete the crew. A 5,000 gallon tank was already in place from a previous job. The first tank to be installed was placed on the east side of the excavation and secured. The second tank was installed 3 ft west of the first tank. The tanks are 27 ft long, 8 ft in diameter. Employee #1 and #2 were between the tanks when a cave-in occurred on the west side and the dirt pushed one tank against the other, crushing and killing both men.
