Investigation Summary
At 4:30 a.m. on May 12, 2017, an employee was operating a plastic extrusion line and was in the process of scraping some stock. The employee crawled underneath the scraper when some stock jammed at the edge of the lower dust pan on the cen ter edge cutter. The employee pushed the material up to clear the jam from the edge cutter saw and had their left hand pulled into the rotating saw blade. The employee amputated the left index finger and middle finger tips when struck by the saw.
