Investigation Summary
Two employees were phasing out a newly installed 7200-volt (phase-to-ground) electric switch. While the employees were using phasing rods and a meter to test one of the phases, an electrical fault occurred. The ensuing electric arc burned both employees. One of them was critically burned with third-degree burns to his chin, neck, arms, and upper body. He was hospitalized for his injuries. The second employee was treated for burns to his face and upper arms and was released. No personal protective equipment had been worn during the operation.

Investigation Summary
Some employees were deactivating a high-voltage load-break switch. An apprentice electrician who was not qualified to work on this equipment was assisting in the operation. The load-side lugs on the switch were deenergized and locked out. This deenergized the load-side lugs but allowed the line-side busbar to remain energized. The rear panel for the switch was removed so the employees could secure the conductors after they were removed from the load-side lugs. When the panel was removed, the 13.2-kilovolt, line-side busbars (7200 volts phase to ground) were exposed to contact. The apprentice electrician went to the back of the switch to get a better hold on the wrench he was using to loosen the lug bolts. As he reached into the cabinet, he contacted an energized part and received an electric shock. His movement caused his hard hat to move close enough to the busbars to create a phase-to-ground fault. The ensuing electric arc ignited his shirt and caused it to melt. He was hospitalized with second- and third-degree burns to his upper torso.

Investigation Summary
Two power line workers were excavating for the installation of an underground power line. At the time of the accident, they were using a metal digging bar. The employees were not using insulation or other protective equipment, nor had they attempted to locate any existing power lines. The metal bar pierced a 7620-volt underground cable and caused an electrical fault. The employees were burned by the ensuing electric arc. One of them sustained burns to his arm, hands, and face. He was hospitalized for his injuries. The other employee sustained burns to his mouth and eyes.

Investigation Summary
An employee (employee #2) was removing a grounding strap from a 4160-volt air switch. The uninsulated handle of the strap contacted the line side of the switch stab, creating an electrical fault. The employee was burned over 50 to 70 percent of his body by the ensuing electric arc. Employee #1, who was working next to him, was also burned by the electric arc and had his clothing ignite. Three other employees sustained burns when they tried to extinguish the burning clothing. The two employees burned by the arc and one of the other employees were hospitalized for their injuries.
