Investigation Summary
At approximately 5:30 p.m. on February 25, 2000, Employee #1 was using a rag to clean the rollers on an extractor machine. The slowly-running rollers caught the rag and his right hand, pulling them into the machine. He sustained crushing injuries to the tips of his index and middle fingers.
