Investigation Summary
Employee #1, of Conex Freight Systems, Inc., Compton, CA, was working at a shipping dock when his supervisor summoned him for help. Employee #1 found the supervisor trying to release the pin latch on the fifth wheel of Yard Goat #103. The latching system was in the locked position and would not couple with the pin on a 40 ft trailer. The supervisor instructed Employee #1 to continue prying the latch system while he started the engine to give it more air pressure. Employee #1 was standing on the rear bumper, which was facing the trailer in anticipation of being hooked up, using a crow bar to loosen the latch. When the supervisor started the truck, it immediately moved backward and knocked Employee #1 onto the fifth wheel hook-up plate and under the trailer's nose. His right leg was severed below the knee, and his left leg was crushed and partially severed. The truck had been left in reverse gear, the brakes had not been set ,and the wheels had not been not chocked.
