Investigation Summary
On September 4, 1991, Employee #1, an asphalt tester, was kneeling with his back to a semi-tractor-trailer rig with a belly dump type trailer loaded with asphalt. Employee #1, who was preparing to make a test, was approximately 10 to 15 ft from the back of the truck. When the operator backed up to dump his load, he backed over Employee #1, who was killed.
