Investigation Summary
At approximately 5:15 p.m. on August 28, 1990, Employee #1 and two coworkers were resetting a mobile grading rack at molder #2. As they pushed the rack toward the molder, an extension cord that had not been unplugged caught on one of the overhead fluorescent lights. The suspending chain broke on one end and the four-tube, 8 ft long light swung down and struck Employee #1's left hand, which was on the rack. The sharp corner caused a deep laceration at the base of his thumb and shattered the thumb's knuckle. He underwent surgery at Rideout Emergency Hospital and was released 26 hours later. Employee #1 had only rolled the rack about 3 ft when he was struck by the falling light fixture. This was the first time he had helped move the rack.
