Abstract: At approximately 1:20 a.m. on July 14, 2017, Employee #1 was working at a site that produced frozen food items. He, a supervisor and a coworker, all maintenance workers, had dissemble an APV heat exchanger for cleaning. After cleaning the heat exchanger, the employees started to assemble the heat exchanger back together. The heat exchanger was used to cool burrito filling before it is extruded onto tortillas. Employee #1 was holding up the female part of a cylinder coupling for the heat exchanger cylinder as the other employees guided the male coupling part into the female part from about six feet away. The cylinder coupling was approximately 62.5 in above the floor and the cylinder was supported by a hoist. While moving the cylinder coupling, Employee #1's left hand slipped and his middle finger became caught between the male and female parts and was amputated. Employee #1 was transported to a medical center, where he received treatment on the finger amputation and was then hospitalized.
