Abstract: At 3:00 p.m. on May 2, 2017, An employee was trying put a cable back on to a pulley. The employee placed his hand into the pulley assembly to get the cable back on the pulley. A coworker actuated the up/down air lifter to relieve tension on the cable, as the employee pulled on the cable causing the mixer to come down, severing the tip (last knuckle) of the Employees index finger on his right hand.
