Abstract: At approximately 3:00 p.m. on July 5, 2002, an employee who worked on a waterline maintenance crew completed the installation of an 8-in. water main. He stood in a 36-in. deep excavation, sealing and abandoning the preexisting waterline. The employee used a 7-lb. sledgehammer to tap a tapered, 6-in. by 8-in., redwood plug into an abandoned waterline. He began to weld a steel retaining strap over the pipe and plug assembly end. An unforeseen build-up of air pressure in this section of the steel water pipe caused the redwood plug to shoot out of the pipe. The flying plug struck the employee on his left ankle, fracturing it. He was transported to San Gorgonio Hospital, where he was hospitalized for 5 days.
