Abstract: On August 23, 2001, Employee #1 of Swinerton Builders was assigned to remove a wooden access bridge that was placed over an excavation that was over 10 ft in depth. The excavation was not otherwise guarded. Employee #1 had removed all guard rails from the bridge and most of the plywood surfaces. He had one 2-ft section of plywood left to remove and then the support beams. As he was removing the last plywood section, the bridge shifted and the beams fell into the excavation, along with Employee #1. Employee #1 had been standing on the land side of the bridge, at the edge of the excavation and was not using any form of fall protection. Employee #1 had numerous fractures to his left arm and was hospitalized for 4 days.
