Abstract: At approximately 12:00 p.m. on January 30, 2014, Employee #1, a carpenter, and Coworker #1, a foreman, both employed by Weslar Inc., a construction framing contractor, were working at a residential worksite. Employee #1 was working from an 8-foot Orco Green Bull A-Frame Ladder while installing a wood beam that measured 14 feet by 1 foot by 5.5 inches. Employee #1, who was standing on the third or fourth rung of the ladder, fell to the ground. Emergency services were called, and Employee #1 was transported to the hospital, where he was admitted and treated for a skull fracture. The employer notified Cal/OSHA at 3:45 p.m. on January 30, 2014. The subsequent investigation determined that the wood beam had been placed on top of 10-foot walls without temporary bracing. Employee #1 had been holding the beam in place while Coworker #1 nailed the beam into the hanger on the opposite side of the beam.
