Abstract: On October 8, 2012, Employee#1 was assigned to install top plate and bottom plates to the floor for the exterior walls and the interior walls on the second floor of the house being framed. Employee #1 had worked his way to the front of the house over the garage area. Employee #1 was almost done with the plating, at the edge of the second floor. Employee #1 placed a 2 ft x 4 ft at the edge, but he did not tack it down because he had run out of nails. Employee #1 was holding a skilsaw in his right hand and a piece of wood in in his left hand, preparing to cut to size. Employee #1 stepped with his left foot on to the bottom plate which was not tacked down, and lost his balance and fell off the edge of the floor, to the concrete area below. This fall resulted in a fractured tail bone, right elbow bruised and swollen, and laceration to his back, where Employee #1 landed on his hammer. Employee #1 was hospitalized for three days.
