Abstract: On December 10, 2012, Employee #1 was applying painter's tape on a wall of a stairway, which was located in an apartment complex. He was using an extension ladder to access areas on the wall. As he was taping the corners, he lost his balance and fell approximately 10 ft. Employee #1 struck a concrete floor and suffered a wrist fracture, back and head injuries. Emergency medical personnel were summoned. They arrived and transported him to a medical center. At the hospital, Employee #1 underwent surgical procedures; afterwards, he was hospitalized for postoperative treatment.
