Abstract: At approximately 3:00 a.m. on May 3, 2012, Employee #1 of G&H Marble was injured. Employee #1 was using a 5" Makita Grinder (9565 CV, serial number: 76743 A) to cut a slab of marble. The cutting blade broke and the broken pieces seriously injured Employee #1's left eye and face. Two employees were installing counter top for Panda Express restaurant located inside a mall. At the time of accident, the grinder Employee #1 used had no safety guard. Employee #1 was not provided personal protection equipment such as goggles or face shield. Employer had no IIPP, did not inspect the grinder periodically and did not provide training to employees.
