Abstract: At approximately 3:47 p.m. on May 26, 2011, an employee was instructing and demonstrating how to operate the acetic acid distribution system to a new coworker. The product line running along a wall overhead ruptured causing a stream of the acetic acid to splash onto the employee. According to statements gathered during interviews and the employer's internal accident investigation report, a 0.375 inch plastic tubing line of the acetic acid distribution system had been removed from its original location, moved and then spliced to another section of the tubing on a different wall. A plastic insert was utilized to seal the gap between the two tubing pieces and clamped in place. As the distribution system was turned on by the employee, the spliced area of the tubing failed, resulting in the rupture and leak of the acetic acid. Neither the employee nor the coworker were wearing any eye and face protection at the time of the leak. The employee sustained an injury to his left eye after being splashed by a heavy stream of Tsunami sanitizer (acetic acid) while attempting to operate a food sanitizing distribution system. The employee was not hospitalized.
