Abstract: At approximately 7:00 a.m. on October 30, 2008, an employee and a coworker were assigned to go up on 15 feet high roof to paint the rim around roof edge. They were lifted on the roof by a forklift. According to the employee's statements, he and the coworker had to lay flat at the roof edge and bent down to paint the rim. There was no fall protection provided at the time performing the task. It started raining toward to the end of the work, so the employee and the coworker prepared to get off the roof. While walking back to the other side of building, the employee stepped on a skylight, three feet by eight feet, and fell through it to the concrete floor. The employee suffered broken left wrist, fractured left face, right ankle, right hip, and back injury. The employee was taken to Harbor UCLA Hospital and was released the next day. A few days later, the employee had his left wrist surgery and was hospitalized for two days.
