Abstract: At 11:00 a.m. on May 29, 2008, Employee #1, who was employed by a family-owned business that rebuilt and sold electrical motors, was working in the back area of the shop, adjacent to the paint booth. Employee #1 was aligning an electrical pump and impeller, which were placed on top of a metal cart. As he was tapping the impeller housing with a rubber hammer, the electrical pump slid off the cart and landed on his right foot. As a result, Employee #1 sustained an amputation of his right toe and was hospitalized from May 29 through May 31, 2008, at Olympia Medical Center. Causal factors were that Employee #1 disconnected the electrical housing from the hoist and was not wearing suitable working shoes, such as steel toe shoes.
