Abstract: At approximately 8:20 p.m. on March 13, 2010, Employee #1 was surveying steam traps, to determine the need for replacement or servicing. The employee was checking out steam traps near a 150 ft trench used to house piping for sulfur. The employee fell into an unguarded section of the trench (39 in. long by 36 in. wide by 61 in. deep). The trench is normally covered by grating. The grating on the southern most section of the trench had been removed creating the pit that the employee fell into. The 48 in. long by 43 in. wide grate covering that section of the pit had been removed so that the condensate from leaking steam tracing on a 6 in. sulfur line could be pumped from the pit. The pit was partially filled with hot condensate or hot water, 300 degrees Fahrenheit. Employee #1 sustained second and third degree burns and was hospitalized for thirty days.
