Abstract: At approximately 7:20 a.m. on January 4, 2008, Employee #1, electrician, entered Building E at the Long Beach City College Campus Center in Long Beach, California. Employee #1 used kitchen's North exterior door, to get to the bathroom on the West side of building. As Employee #1 left the Building E's kitchen, he turned West and entered the Fishbowl, where all the lights were turned off. As Employee #1 cautiously moved through the dark Fishbowl towards the bathroom, he came upon a cart laying in his path way. He tripped over it and fell onto his left shoulder. After falling, Employee #1 got up and called his supervisor, informing him that he fell and got hurt in Building E. Employee #1's supervisor found Employee #1 at Building E. Paramedics of Long Beach Fire arrived at Building E and treated Employee #1's injured left shoulder. After that, Employee #1 was transported by his supervisor to Harriman Jones Medical Clinic, where he was diagnosed with fractured left humerus. Then, Employee #1 was transferred to the Long Beach Memorial Hospital, where he underwent surgery on his left shoulder.
