Abstract: On August 24, 2011, Employee #1 was testing the Alpina Machine, a sausage mixer. She was checking to make sure the metal detector was working. She placed a blue 2-in. plastic coated metal test object in the Alpina Machine to make sure that the metal detector will shut off the machine so that not any metallic items will contaminate the sausage. The metallic object got stuck in the Alpina Machine and the employee tried to pull-out the metallic test object without shutting the machine down. She placed her index finger into the machine when she triggered the machine, and it amputated her left index finger tip. The employee was transferred to Stanford Hospital, where they reattached the finger. Employee #1 was hospitalized. The tube where the employee stuck her finger had been lengthen and a pneumatic shut-off had been installed and the employer had had no incidents nor actions since this accident.
