Abstract: At approximately 5:15 a.m. on June 4, 2009, Employee #1 was a salesman. He was checking a gear box on a sugar silo machine to verify the information needed for an order of parts. He reached around the chain and sprocket guard into an area where the guard was not flush with the machine. His right hand became caught in the chain and sprocket, amputating two fingers. He was hospitalized to treat the amputation.
