Abstract: At approximately 7:30 a.m., on May 4, 2012 Employee #1 was seriously injured. They had installed ply-decking to the outside decks, on the day of the accident Employee #1 had been assigned to buff or smooth the deck surfaces. Employee #1 was assigned a stone wheeled grinder (Hilti DG-500-D) from his supervisor, which he inspected and found to be in good condition. He used the grinder to do 2 decks and was working on the third deck when the stone grinder wheel exploded into several pieces. He suffered an acute laceration and fracture to his left knee as a result of the accident. Employee #1 was taken by ambulance to UC Davis Medical Center in Sacramento CA where he was hospitalized for greater than 24 hours for surgery and treatment of an injury to his left knee. Employee #1 had been trained to use the equipment and was using all of the required PPE at the time of the accident. The equipment was not defective and was being used correctly.
