Abstract: On June 16, 2008, Employee #1 had finished adjusting the fly chain of a motorized asphalt spreading machine. Because the fly chain cannot function properly when it is too tight, Employee #1 engaged the fly chain to watch it operate. When running, the fly chain moves a conveyor belt in a continuous loop and exposes employees to the chain, belt, and attached metal bars installed across the belt's width. At the end of the loop closest to Employee #1, the belt and metal bars pass through an area that is just large enough for clearance. As Employee #1 watched the moving conveyor, he was standing with one foot on the frame of the machine. While in this position, Employee #1 touched the conveyor belt and metal bars to test the tightness. Employee #1's foot slipped off the frame and he instinctively grabbed one of the moving metal bars for support. The metal bar immediately drove Employee #1's hand into the narrow space at the end of the conveyor belt loop, causing injuries to his right hand. Employee #1 sustained sprains and strains and was hospitalized.
