Abstract: On March 18, 2008, an employee, a roofer, was using an all terrain forklift to load roofing on the roof at a residential home. The employee drove the forklift up a slope at the building to off load the roofing materials. For an unknown reason, the employee back down the slope at an angle, this placed the forklift at a horizontal on the slope. The forklift truck rolled onto its side as the employee was backing down the slope. The employee was wearing his seat belt. The employee's left foot and ankle were pinned beneath the forklift truck. The employee was taken to the hospital treated and released.
