Abstract: On February 14, 2008, Employee #1 was working in the receiving department of an ice cream manufacturing facility. While Employee #1 was pumping milk from a tank truck to the plant system, he failed to secure the valve (plunger) on the tank truck. When he realized that the valve was not secured, he immediately placed a block on the valve stem to prevent it from closing. The valve snapped shut, catching the tip of his ring finger on his left hand. Employee #1 was taken to a nearby medical facility, where he was hospitalized and treated for his injury that required the amputation of his left ring finger
