Abstract: At approximately 12:30 a.m. on February 19, 2013, Employee #1, a 27-year-old male, machine operator, with Transilwrap Company Inc., was working on the Kampf Slitter Machine when the plastic web broke. Employee #1 tried to push the plastic between the moving rolls of machine when his left fingers and forearm were pulled into the rolls of machine. Employee #1 suffered a fractured left wrist and crushed fingers, when his fingers/forearm were caught in the moving rolls of a polythene slitter rewinder located in production department. Employee #1 was transported to Arrowhead Medical Center, Colton, California, and hospitalized overnight due to injuries. The Investigation revealed that the existing plastic guard in front of the rolls of Kampf Slitter Machine was inadequate for protection as it did not extend all the way down to cover the rolls completely except for a thin opening to allow the plastic film to be fed through. The unguarded portion in front of rolls was measured to be 93 in. long by 5 in. wide at inspection time. The Employer was engaged in polythene film distribution since July 2011 and employed 32 workers. Employee #1 had been employed about three months as a machine operator when the incident occurred.
