Abstract: At approximately 7:35 a.m. on September 4, 2007, Employee #1, a full-time employee who had been employed for 2.5 months as a millworker in the production department at Forest Plywood Sales Inc., was manually moving the lumber cart in the production area. The size of cart was measured to be two feet high by five feet wide. According to the employer, Employee #1 was manually moving a lumber cart with some lumber pieces at high speed in the production area, when the cart tipped and crushed his right middle finger between the floor and cart. Employee #1 suffered a partial amputation to the tip of his right middle finger. He was hospitalized for treatment.
