Abstract: On September 2, 2010, Employee #1, who worked for the gas company, was assigned to trim tree branches. He climbed an A-frame ladder (Warner 8-ft, 225-lb capacity, Model Number 6343-A331, Part Number 51755-01) to cut the branches with a pair of hand cutters. Employee #1 did not have a three-point contact with the ladder, and he was not facing the middle of the step or rung of the ladder while overreaching overhead to cut the tree branches. He fell backwards from the A-frame ladder and landed on his back, striking a metal parking bar adjacent to the wall. His coworkers came to assist him and called the paramedics. Employee #1 was taken to Encino Medical Hospital, where he was diagnosed with four fractured vertebrae and three fractured ribs. He was hospitalized six days for treatment.
