Abstract: At approximately 12:00 p.m. on April 19, 2010, Employee #1 and a coworker were installing a new plate for a generator. The previously installed generator was not bolted correctly to the base plate that it sat on. The coworker was using a Hi-Lift jack (Model Number XT-485) to raise the 2,200 pound generator, and as he cranked the jack once, the generator was raised approximately one inch. The mechanism of the jack causes it to lower approximately 0.625 inches when it was cranked, so that the total lift height after two cranks was approximately one inch. After the first crank, Employee #1 placed his hand under the load to push a plate out. The coworker was not aware that Employee #1 placed his hand under the load, and cranked the jack again, causing it to come down on Employee #1's left hand. As a result, Employee #1 sustained an amputation of his left hand middle finger tip. Employee #1 worked for Motive Energy, Incorporated, a company that engages in the installation of back-up generators for the telecommunications industry.
