Abstract: At approximately 2:00 p.m. on June 17, 2010, Employee #1, the owner of Toste Grading and Paving, was for some reason standing behind an asphalt truck. The truck driver, an employee of V&J Trucking, backed up the truck and ran over Employee #1. Employee #1 was killed. The inspection was reopened on June 22, 2010. It was determined that Employee #1 was walking in and out of a Peterbilt truck and trailer when the truck driver, unaware of Employee #1 whereabouts, pulled forward. Employee #1 was run over and crushed. There were no witnesses to the accident.
