Abstract: At approximately 3:58 p.m. on February 5, 2008, Employee #1 was a mechanic. He was repairing a prime mover conveyor at a citrus packing and shipping facility. He had locked out the equipment to perform the repair. He replaced a master link on chain and sprocket assembly on accumulator belt, and reenergized the equipment. He had left a channel lock pliers on the accumulator belt. He was injured when he reached for the pliers and his left pinkie finger became caught in the chain and sprocket. He was hospitalized for treatment of the amputation of the distal finger joint.
