Abstract: On August 21, 2007, Employee #1 was told to operate a "berry hoop making" machine. Employee #1 had never previously operated this machine and was told how to operate it but never shown how to operate it. There was no guard on the machine at the time of his accident. The machine consists of several pulleys. The employer grows and harvests berries. In the fall and winter, the berries have to be protected from rain and wind. The employer places a protective arched barrier over the plants. Employee #1 placed a long metal tube horizontally on an end rest and feeds the tube by hand into the machine. As it enters the machine, there is a pulley approximately 0.5-in. to approximately one inch from the opening of the guard. The opening is approximately 4-in. by 4.5-in. square. However, at the time of accident there was no guard on the machine, exposing Employee #1 to a revolving pinch point hazard. Employee #1 was looking away from the machine when his hand got too close to the first pulley and got caught. As the tube is being fed, another employee operates the side controls which move another set of pulleys to form a bend or arch. Employee #1 suffered a partial tip amputation of one finger of his right hand. Employee #1 was hospitalized.
