Abstract: At 10:30 a.m. on July 19, 2007, Employee #1 and two coworkers were assigned to move surplus jack equipment from a connex trailer. The employer was Lockheed Aeronautics, an aircraft manufacturer. No supervisor was present during the incident. Employee #1 and the first coworker were lifting a tool cabinet within the connex trailer with a pray bar extension. A second coworker was operating a forklift (BT Prime Mover, model number CSX40, serial number CSX40282/003) to place the forks underneath the cabinet to lift it while Employees #1 and the first coworker were prying the cabinet from underneath with the extension bars. When the second coworker moved the forklift forward, it pinned Employee #1 between the left side of the connex trailer and the forklift guard. The second coworker quickly reversed the forklift and then Employee #1 said that he might have broken bones. Employee #1 was medically evacuated to Holy Cross Hospital where he was diagnosed with broken ribs, a broken shoulder blade, a punctured lung, and a swollen right leg. Employee #1 was in the hospital for 7 days.
