Abstract: On August 16, 2006, Employee #1 was using a self-made sleever bar made out of rebar, to align the holes on a channel beam to the holes on a flange. The flange was welded to a carrier beam. He was sitting on top of an I-Beam (carrier beam) approximately 13 ft high, to the left of the crane operator's view. The crane operator observed a coworker, on the opposite beam, having a problem setting the beam and getting hung up on the flange. Something was cut off, either part of the flange or part of the I-beam making the channel beam jerk. This jerking caused Employee #1 to lose his balance and fall. He fell head first the anchor bolt that was securing the vertical steel column. Employee #1 was killed.
