Abstract: On December 1, 2005, Employee #1 and a coworker were applying "black insulation paper" to the outside of an apartment unit, which was being remodeled. Each employee was working from a 20-ft extension ladder, holding portions of the paper and stapling it to the building's framed walls. After completing the lower sections of the building, the two employees proceeded to the upper section, where the roof height was approximately 23 ft. The coworker climbed to the roof, while Employee #1 climbed approximately 20 ft on his ladder. The coworker then handed Employee #1 the paper to apply, when the ladder on which Employee #1 was standing on, began to slide off the building. Employee #1 fell to the ground landing on the concrete walkway. He sustained a fractured left wrist and injuries to his back and right knee. Paramedics took Employee #1 to Northridge Hospital, for surgery to his left wrist. He was hospitalized for two days. Employee #1 subsequently had surgery on his right knee at another hospital in February 2006.
