Abstract: At approximately 9:20 a.m. on September 5, 2005 Employee #1 was working for a small painting contractor. Employee #1 was the son of the owner and had been working for the company for approximately seven months. Employee #1 was preparing surfaces for painting and setting up equipment for painters. At the time of the accident Employee #1 was assigned to "razorblade" the surface of the exterior faces of the second story windows. Employee #1 had setup a 32-ft Werner extension ladder. There were no witnesses to the accident. Employee #1 was found on the ground but the ladder position had not moved. Employee #1 did not recall the events that lead to the accident. Employee #1 was hospitalized for four days and received treatment for head injuries.
