Abstract: At approximately 7:15 a.m. on August 2, 2005, Employee #1, a machine operator, was working at a large manufacturer of transportation related components, manufacturing ball lock pins. She was operating a Bliss C-22 power operated press. The press notches a rectangular hole on the face of a small cylindrical metal tube. She was retrieving a newly punctured part from the point of operation when the top die assembly, without operator intervention, initiated the down stroke. Due to the lack of a brake monitoring device a problem with the brakes went unrecognized. The top die bottomed with her hand in the point of operation. She received a crushing injury to her right middle finger. She was hospitalized and her finger was amputated.
