Abstract: At approximately 1:50 p.m. on February 2, 2006, Employee #1 was working on top of flat top roof trusses during installation, when one of the trusses broke apart at a joint. He fell 16 feet, 1 inch to the concrete floor and was killed. An investigation revealed that the trusses were not adequately braced and were mishandled during installation. The external stresses on the truss and the damage caused by improper handling caused the truss to fail at a joint.
