Abstract: On March 11, 2011, Employee #1 was working for a company that specializes in the distributing and packaging of vitamin supplements. He was working in the production room, using an auger feeder to mix a product called "Alkachofa" when a jam occurred. As he reached into the auger with his right hand to clear the jam, his right thumb contacted the auger and was amputated. Emergency services were contacted and, upon their arrival, Employee #1 was taken to County USC Medical Center, where he was hospitalized and treated for his injury. The accident investigation determined that workers failed to use an extension tool when attempting to clear a jam. The employer was cited for a serious violation for failing to provide and ensure the use of a suitable extension tool when exposed to direct contact (e.g., clearing a jam) with the moving auger.
